In celebration of our opening, we are offering school
pre-participation physicals for $25.
For your convenience, you may print out forms
ahead of time and be ready for your visit.
UIL Physical Form - Can be filled out online and printed
Tomball ISD Physical Forms - Must include above form as well
If indicated, we can schedule at our office:

EKG
- Echocardiogram (Ultrasound of the heart)
- Spirometry (Assesment of asthma severity and lung function)
- CMET - cardiopulmonary stress test which provides dynamic information on heart function, lung function, level of fitness, and customized fitness plan.
