Patients that are new to Family Health Pro must fill out and print the 
New Patient Registration and Notice of Patient Responsibility.
Note: Patients who have seen Dr. Valdez before, but not at this office,
must fill out the Registration and Notice of Patient Responsibility.

When you visit your doctor, it's important that he or she knows all about your medical condition.
This questionnaire will help your doctor give you the best care possible.
You will be asked questions about your medical history and your current symptoms.
Your medical information is confidential and will only be shared with your doctor.

All patients over the age of 18, and regularly prescribed controlled medications
such as pain medications (hydrocodone, opiates), muscle relaxers (soma),
benzodiazepam (xanax, valium, ativan, etc), or should review our office policy
as well as our contract.
